It is vital to keep your personal information up to date so you can keep your care active. It also helps make sure you get updates and reminders from us. Your personal information is your:

• Phone number
• Email
• Mailing address

Step 1: Make sure Molina has your info.

Log in to the My Molina member portal or use the My Molina mobile app to change your information in the Communication Preferences section. Or call Member Services at (888) 999-2404 (TTY: 711).

• Confirm receipt. Look for a text, email or letter that your changes were made. Or log in to My Molina member portal or mobile app to make sure your profile shows your new info.

Step 2: Make sure ForwardHealth has your info.

This helps keep your care active. There are three ways you can do this:

• Online at ACCESS. Log in to change your information.
• By phone at (800) 362-3002 (TTY: 711).
• Call or visit your local income maintenance and tribal agency: To find the nearest place, go to Income Maintenance and Tribal Agency Map.

Need help in your language? If you opt for talking or reading in your own language, you have the right to ask for language support at no cost. This involves interpreters and information written in your preferred language.

A translator can help you:

• Schedule a visit
• Talk with your doctor or nurse
• Get emergency care
• File a complaint, grievance or appeal

How to get a translator

• If you have a visit your doctor can help you find a translator. Call us Monday-Friday, 8 a.m. – 5 p.m. local time at (888) 999-2404 [TTY: 711]. Ask to speak with a translator.

Translator Request Cards

Download and take one of these cards with you when you go to the doctor. It will help them know what language you speak. It says: Hello, I speak (say your language). I need an interpreter. My health plan, Molina Healthcare, will pay for an interpreter if you do not have one. Please call (888) 999-2404 and state: “I have a Molina patient and I need an interpreter.”
