How Medicare eligibility works
To join a Molina Medicare plan in California, you must live in our service area and have Medicare Part A and Part B.
For our D-SNP plans, you must also be in a qualified dual-eligible plan. You, Medicaid or another party must continue paying your Medicare Part B premiums.
Here’s what you need to apply:
- Your Medicare number and Part A and Part B start dates
- Your Medicaid information if enrolling in D-SNP
- Your home address and contact information
