Make sure you live in the area our plan covers and have Medicare Part A and Part B. For D-SNP, you must also have Medicaid and be in a qualifying dual eligible grouping, See your Evidence of Coverage booklet for qualifications for each plan.
Apply online, by mail or through Medicare.gov. For D-SNP, have your Medicare and Medicaid info ready.
D-SNP members must fill out a Medicaid form once a year to keep their health plan. This form helps the state check if you still qualify for Medicaid. You should look for notices and complete any steps on time so your plan does not end. If you keep your Medicaid eligibility active, your Medicare duals plan renews by itself.
Enrollment is handled and approved by the Centers for Medicare and Medicare Services (CMS). If CMS does not accept your enrollment, we will let you know.
To join a Molina Medicare plan in Iowa, you must live in the area where we have a health plan and have Medicare Part A and Part B. For Molina Medicare Complete Care (HMO D-SNP), you must also have Medicaid and be in a qualifying dual eligible grouping. See your Evidence of Coverage booklet for the qualifications for each plan. You, Medicaid, or another party must keep paying your Medicare Part B premiums.
