Molina Healthcare’s Healthy Actions Rewards Program was made just for our members. We want to support you on your health journey. See your doctor, complete your healthy actions and get rewards with Molina.
What Are Healthy Actions?
Healthy actions are routine health exams you complete with your doctor. These visits are vital to keep you healthy or catch health problems early. They help your doctor come up with a care plan for you. Complete your healthy actions with your doctor between 1/1/2026 and 12/31/2026. Get your rewards and let us know what health exams you completed by 1/31/2027. Use your rewards by 3/31/2027.
You can earn rewards in the form of a pre-funded debit card when you visit the doctor and get shots.
When you finish your health exams, you can get these rewards:

Yearly diabetes testing for adults ages 18-85 with diabetes. You must complete all 3 screenings to claim your reward.

Your rewards will be loaded onto your Benefits Mastercard Prepaid Card. You can use this card at participating retailers. Visit the Nations Benefits Portal for a full list of retailers and eligible items.
What Happens to my Rewards if I am no longer part of the Molina plan?
You must be an active Molina member to get your rewards. You will lose your unspent rewards if you are no longer part of the Molina plan.
If you have questions, call the Wellness Team.
The rewards program may change based on CMS or state rules. Members must complete the approved service before getting a reward. Molina may call you to make sure you completed your visit.
Disease management programs
Molina offers Disease Management Programs that can help you handle your life-longhealth issues. Our programs offer written info, help and care tips. We also help you work with your doctor.
Members are found through health care or pharmacy claims. Members are enrolled in the program(s) that meets their needs. If your health issue is more complex a Nurse Care Manager will call you. The nurse works with your doctor to get you the care you need. If you have a life-long health issue not listed below you may call us. We will send you to a Nurse Care Manager for help.
There are many ways you can be referred to these programs. One way to sign up is through your doctor. You also can call us and say you want to be in the program. There are certain requirements that you must meet. These programs are voluntary. You can choose to be removed from a program at any time.

Healthy Living with Diabetes® program
This program is for adults ages 18 and above living with diabetes. You will learn:

How to work with your main doctor and care team to handle diabetes.
Living with Chronic Obstructive Pulmonary Disease (COPD) program
This program is for adults with COPD. You will learn:

About working with your doctor to keep your COPD under control
The warning signs of COPD
How to take your meds as ordered
Heart Healthy Living program
This program is for adults with:
High blood pressure
Coronary artery disease and/or congestive heart failure.
You will learn:
About coronary artery disease, high blood pressure and/or congestive heart failure
About taking your meds
About making health changes to quit smoking and eat healthier
Virtual Care
Virtual Care means:
• Getting care online or phone visits, in any place.
• You can get care at no cost.

Talk to a nurse 24/7 day or night, by calling our 24-hour Nurse Advice Line.
Virtual care
Virtual care through telehealth services – search the Molina Provider Online Directory to find a PCP who can give you care online or over the phone.

Case Management
Living with health problems and handling them can be hard. We offer special programs to support you.
This program is when a nurse works with you and your main doctor. The nurse will help you get the care you need.
We also offer Complex Case Management programis for members with health needs that are harder to handle. Both programs allow you to work with a nurse. The nurse can help you learn more about your health problems. They can teach you how to better handle your health problems.
There are many ways you can be referred to this program. One way to sign up is through your doctor. You can call us and ask to join the program by calling Member Services. There are certain requirements that you must meet.
This program is voluntary and is offered at no cost to you. You can choose to be removed from the program at any time.
Care Coordination
Molina offers this support to help you with long‑term health needs. This health problem can be either physical or mental.
A Care Coordinator’s job is to:
Help you find care that are not part of the Molina plan such as Meals on Wheels.
Help you use care that you may not be using under your plan.
Help arrange visits and tests.
Help arrange a ride.
Help find support if you are a person with special health care needs.
How can I help my Care Coordinator help me?
Write down your questions. This will help you get all your answers when you have a lot to talk about.
Tell your Care Coordinator about all your visits and tests. If they don’t know about each of your visits and test, it is harder to help you.

Getting a Ride
We give rides to health care visits, as well as doctor’s visits, labs, x‑rays and therapy.
Getting a ride when it is not a crisis does not need prior approval.
To plan a ride, call 2 business days before your visit. Please let us know if you need a wheelchair van or other care.
To cancel your ride, please call 24 hours before of your visit.

Over-the-Counter (OTC) Benefit
Your plan includes non-prescription OTC health items like:

The Silver & Fit Fitness Program
You can join a local fitness club or get a home fitness kit at no cost. A caregiver may join you to help you feel safe.

You can call the 24‑hour Nurse Advice Line for help 24 hours a day, 365 days a year. Nurses can look up trusted health information and guide you based on your health issues.You can talk to a nurse in your language about any health problem.

Health education programs
Molina has many programs and care to offer you. There are many ways you can be referred to a program. One way to sign up is through your doctor. You also can call us and ask to join the program. There are certain requirements that you must meet. You can choose to be removed from these programs at any time.
You can stay healthier when you ask for help with:
This program is for members who are ready to kick the smoking habit. The program is for smokers 18 years of age. You will be asked about your eagerness to stop smoking. If you are ready, you may be eligible to join. The program is done over the phone and lasts for six months.
You will be mailed a kit with written info to help you stop this habit.
A stop-smoking coach will call you to offer support.
You will also be given a phone number that you can call when you need help.
Weight Control Program
This program is for members who may need help handling their weight. The weight control program is for Molina members. You will learn about healthy eating and workouts. You may go to classes in your area if classes are offered.
SOLERA HEALTH | MEDICARE DIABETES PREVENTION PROGRAM
Molina Healthcare has partnered with Solera Health to offer the Prevention Program.
This program helps match you with support that fits your needs. This program can help you build healthy habits and lower your risk for type 2 diabetes.
The program may include:
A focus on healthier food choices and greater activity levels
16 weekly lessons over the span of six months, followed by monthly support sessions
Small group, in-person classes for support

Women's Health Care Services
You can get women's health care doctors who work with Molina Healthcare. You do not need a referral from your main doctor. This may involve care such as:
Pap tests
Mammograms
Family planning
Motherhood Matters® Pregnancy Program
This program will help pregnant women get the schooling and care needed for a healthy pregnancy. You will be mailed a workbook and other support. You will be able to talk with a Care Manager about any questions you may have during your pregnancy. Also, you will learn ways to stay healthy during and after your pregnancy.
Preventive Health Guidelines
When to Get an Exam
Do you know when you need an exam or other preventive health care? Our Preventive Health Guidelines outline recommended exams and care for you and your family. They give basic ideas for healthy children and adults without known health care issues. They should not replace your doctor’s advice.
Do your part to stay healthy!
Take a few moments to read the Guidelines to Keep You Healthy. You can also view the Immunization Schedules. The guides suggest exams and shots to keep you healthy. Molina covers health care that prevents sickness.
Write down any questions you may have. Take these questions to your next exam.
Tell your doctor about any health problems you or your children are having.
Make sure you do not miss your visits.
If you miss your visit, call your doctor to reschedule right away.
Effective January 1, 2010, Molina Medicare covers all standard and FDA‑approved rapid HIV tests.
We cover yearly HIV tests for members at higher risk. This includes:
Men who have had sex with men after 1975.
Men and women having unprotected sex with multiple partners.
Past or present injection drug users;
Persons who exchange sex for money or drugs or have sex partners who do;
People whose partners have HIV, are bisexual or use injections
Persons being treated for sexually transmitted diseases;
Persons with a history of blood transfusion between 1978 and 1985;
Persons who ask for an HIV test, even if they do not share risk factors.
We also cover HIV testing for pregnant members. Tests may be done when pregnancy is known, in the third trimester and during birth.

Molina offers the monthly planner at NO COST to you. You can download and print it yourself. The planner has tips on a range of monthly health topics. Use it to keep track of your meds, doctors’ visits or your doctors phone numbers.
