Make sure you live in our service area and have Medicare Part A and Part B. For D-SNP, see your Evidence of Coverage booklet for specific qualifications for each plan.
Apply online, by mail or through Medicare.gov. For D-SNP, have your Medicare and Medicaid details ready.
D-SNP members must apply for Medicaid each year to maintain coverage. If you keep your Medicaid eligibility, your D-SNP renews automatically.
Enrollment is subject to approval from the Centers for Medicare and Medicare Services (CMS). If CMS does not accept your enrollment, we will notify you.
To join a Molina Medicare plan in Michigan, you must live in our service area and have Medicare Part A and Part B.
For our D-SNP plans, you must also have Medicaid and be in a qualified dual eligible group. You, Medicaid, or another party must continue paying your Medicare Part B premiums.
