Healthy Actions Rewards Program
Reach your health goals with Molina Healthcare. Molina Healthcare’s Healthy Actions Rewards Program was made just for our members. We want to support you on your health journey! See your doctor, complete your healthy actions, and earn rewards with Molina.
Disease Management Programs
Molina Medicare offers Disease Management Programs that can help you manage your chronic health conditions. Our programs offer learning materials, advice and care tips. We also help you work with your doctor. The goal is to help you learn to manage your health conditions.
Members are found through medical or pharmacy claims. Members are enrolled in the program(s) that meets their needs. If your health condition is more complex, a Nurse Care Manager may call you. The nurse works with your doctor to help you stay healthy.
There are many ways you can be referred to these programs. One way to enroll is through your doctor. You also can self-refer to the programs. There are certain requirements that you must meet. These programs are voluntary. You can choose to be removed from a program at any time.

Care Management and Complex Care Management
Living with health problems and taking care of them can be hard. We offer special services and programs for members with health problems. Case Management is when a nurse works with you and your Primary Care Doctor (PCP). The nurse will help you get the care you need. Complex Case Management supports members with serious health needs. Both programs allow you to work with a nurse. The nurse can help you learn more about your health problems. He or she can teach you how to better manage them. The nurse will also work with your family and doctor to get the care you need.
There are many ways you can be referred to a program. One way to enroll is through your doctor. You also can self-refer to the program by calling Member Services. There are certain requirements that you must meet.

Personal Emergency Response System (PERS)
Molina Medicare Complete Care (HMO D-SNP) and Molina Dual MI Coordinated Health (HMO D-SNP):
When authorized, we will provide an in-home device to notify the appropriate personnel in the event of an emergency (e.g., a fall).
