Apply online, by mail or through Medicare.gov. For D-SNP, have your Medicare and Medicaid details ready.
Make sure to renew
D-SNP members must reapply for Medicaid each year. If you keep your Medicaid eligibility, your D-SNP renews automatically.
How Medicare eligibility works
Molina Medicare Complete Care (HMO D-SNP) is for people who have both Medicare and Medicaid. To join a Molina Medicare D-SNP plan, you must live in our service area and meet specific qualifications in your Evidence of Coverage (EOC) booklet.
What you need to apply:
- Your Medicare number and Part A and Part B start dates
- Your Medicaid informationif enrolling in D-SNP
- Your home address and contact details

Enrollment is subject to approval from the Centers for Medicare and Medicare Services (CMS). If CMS does not accept your enrollment, we will notify you.
