Have your Medicare info ready. If you are applying for D-SNP, also have your Medicaid details.
D-SNP members must keep their Medicaid each year. If you stay eligible for Medicaid, your D-SNP plan renews automatically.
Enrollment is subject to approval from the Centers for Medicare and Medicare Services (CMS). If your enrollment is not accepted by CMS, we will notify you.

You have Medicare Part A
You have Medicare Part B, and you or Medicaid continue to pay your Part B premium (if needed)
You have Medicaid
You are in a qualifying dual eligible group
