See if you qualify
You may be able to if:
- You live in a Molina service area
- You have Medicare Part A and Part B
- For D-SNP, you must have Medicaid and be in a qualifying Medicaid category (FBDE, SLMB+, or QMB+)
How to sign up
You can sign up:
- Online
- By mail
- Through Medicare.gov
Have your Medicare info ready.
If you are signing up for D-SNP, also have your Medicaid details.
Make sure to renew
For D-SNP, you must also be in a qualifying dual eligible category. See your EOC for specific qualifications for your plan.
How Medicare eligibility works
Molina Medicare Complete Care (HMO D-SNP) is a Medicare Advantage plan for people who have both Medicare and Medicaid.
It has all Original Medicare benefits and more.
You can join if:
- You live in the plan’s area
- You have Medicare Part A
- You have Medicare Part B, and you or Medicaid continue to pay your Part B premium (if needed)
- You have Medicaid
- You are in a dual eligible group that fits the rules (QMB, QMB+, or FBDE)
