How Medicare eligibility works
Senior Whole Health Medicare Complete Care (HMO D-SNP) is a Medicare Advantage plan for people who have both Medicare and Medicaid. It includes all Original Medicare benefits and more. You can join our Medicare Complete Care (HMO D-SNP) if:
- You live in the plan’s service area, which includes the following counties:

- Bronx
- Kings
- Nassau
- New York
- Orange
- Queens
- Richmond
- Rockland
- Westchester
- You have Medicare Part A
- You have Medicare Part B, and you or Medicaid continue to pay your Part B premium (if needed)
- You have Medicaid
- You must be enrolled in our Medicare Advantage program
SWH of New York NHC (HMO D-SNP)
Our NHC (HMO D-SNP) plan is a comprehensive Medicaid Advantage Plan (MAP). To be eligible for our NHC (HMO D-SNP) plan, you must:

Be age18 or older

Be eligible for long-term care

Have both Medicare and Medicaid
SWH of NY MLTC Plan
Our Managed Long Term Care (MLTC) plan provides long-term care benefits and services for adults with Medicaid. To be eligible for our MLTC plan, you must:
- Be age 21 or older
- Be Medicaid eligible
- Live in our New York City service area:

- Kings
- Nassau
- New York
- Queens
- Suffolk
- Westchester
- Have health care needs that can be safely met in your home and/or community
- Be assessed as requiring at least one (1) of the following long-term care services and care management for a continuous period of more than 120 days from the date of enrollment:

- Nursing services in the home
- Therapies in the home
- Home health aide services
- Adult day health care
- Personal care services in the home
- Private duty nursing
- Consumer-Directed Personal Assistance Services
