Healthy Actions Rewards Program
You can earn rewards in the form of a pre-funded debit card when you see your doctor for routine tests and vaccines.
These visits help you stay healthy or find health problems early.
You can earn rewards for:

Screening to check for signs of colon cancer in adults ages 45-75 |

Mammogram to check for signs of breast cancer in women ages 40-74 |

Diabetes screenings |
Yearly diabetes screenings for adults ages 18-75 with diabetes. You must complete all 3 screenings to claim your reward:

How to claim your rewards
There are three ways you can claim your rewards:

Your rewards will be loaded onto your debit card.
You can use this card at participating stores.

You can stay get guided help with:
Pregnancy Program
Get extra support during pregnancy and after your baby is born.
You will receive a workbook and other helpful resources.
You can also talk with a care manager about pregnancy questions.
Silver&Fit® Fitness Program
You can join a local fitness center or get a home fitness kit at no cost. Start here or call (877) 427-4711 (TTY: 711) Monday through Friday, 8 a.m. to 6 p.m. local time.
