- You have Medicare Part A and Part B

How to enroll
You can apply:

- Through Medicare.gov
Have your Medicare info ready.
If you are applying for D-SNP, also have your Medicaid details.
Make sure to renew
D-SNP members must keep their Medicaid each year.
If you stay eligible for Medicaid, your D-SNP plan renews automatically.
How Medicare eligibility works
Molina Medicare Complete Care (HMO D-SNP) is a Medicare Advantage plan for people who have both Medicare and Medicaid.
It includes all Original Medicare benefits and more.
You can join if:
- You live in the plan’s service area
- You have Medicare Part A
- You have Medicare Part B, and you or Medicaid continue to pay your Part B premium (if needed)
- You have Medicaid
- You are in a qualifying dual eligible group
