• Iowa Medicaid
  To enroll in our plan, you must apply for and be eligible for Iowa Medicaid. Visit the Iowa Department of Health and Human Services (Iowa HHS) website to for more information on eligibility and how to apply. If you are eligible, the Iowa Department of Health and Human Services will send you a letter to let you know. Then you can choose Molina.

• How to enroll?
  • Be sure to check your mail for this letter it will include the name of the Health Plan you are assigned to
  • If you are assigned to Molina, you don’t have to take any further action to stay with us
  • If you were assigned to a different health plan and would like to join Molina, you will have 90 days from enrollment to switch your health plan
  • You can also switch plans during a 60-day period known as the Open Choice Period or Open Enrollment
  • Outside of these time frames you must have good cause to switch health plans

  • Here is what you’ll need to have when you talk to a representative:
  • Your annual reported household income
  • Age of the adults in your household
  • Number of dependents
  • Your zip code
• How to switch to Molina
  • Call Iowa Medicaid Member Services at (800) 338-8366 and let them know you’d like to have Molina as your health plan
  • Email Iowa Medicaid Member Services at IMEmemberServices@dhs.state.ia.us.
  • Or Iowa Medicaid Member Services can mail you an MCO enrollment form that you can mail or scan and email back. To request a paper form, call Iowa Medicaid Member Services at (800) 338-8366 or(515) 256-4606
