To be eligible to receive Passport Health Plan by Molina Healthcare benefits, you must be enrolled with the Kentucky Medicaid program.

To find out if you are eligible you can fill out an application online at kynect.ky.gov.

Once you submit your application, the Kentucky Department for Community Based Services will review and determine if you are eligible to receive Medicaid.

 

You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
