You no longer need to carry your Medicare ID card and your Medicaid card when getting medical services. Now all the information you need to show you qualify for benefits is on one card. Your Molina Complete Care for MyCare Ohio ID card will also include important phone numbers and prescription information to help you get your medicine.
You will get your Molina Complete Care for MyCare Ohio ID Card in the mail around the time your enrollment is effective.
You must use your ID card when you get covered services and prescriptions at network pharmacies.
If your ID card is damaged, lost, or stolen, call Member Services. We will send you a new card. You can also request a new card online at My Molina.
