Personal Care I
This service provides assistance with general household activities.
Personal Care II
This service helps individuals with activities such as bathing, dressing, preparing meals, housekeeping, and observing health signs.
This is a member directed service to provide personal care assistance.
Minor home adaptations
This service involves both pest control services and minor physical adaptation to the home.
This service provides skilled medical monitoring, direct care and intervention in order for health care needs to be maintained through home support
Personal Emergency Response System (PERS)
This service provides an electronic device which enables high risk individuals to secure help in the event of an emergency.
Institutional Respite Care
This service provides temporary relief for the caregiver by admitting the member to an inpatient facility (nursing home or hospital).
In-Home Respite Care
This service provides temporary relief for the primary caregiver and maintains the member at home to avoid institutionalization.
Home Delivered Meals
This service provides regular or special diet meals delivered to the member’s home.
Some Nutritional Supplements
This service provides some nutritional supplements.
Specialized Medical Equipment and Supplies
This service provides medical supplies to assist with care at home.
