Note: To join our Iowa Medicaid network, you must first apply online to enroll as an Iowa Medicaid provider and have an active Medicaid ID number.

• Medical, behavioral health and ancillary providers complete a Molina contract request form. Email your completed form to IAProviderContracts@MolinaHealthcare.com.
• Vision providers please contact our vision vendor, VSP, for participation at providerrecruitment@vsp.com or by visiting eyefinity.com.
• Pharmacies offering services billable under pharmacy benefits contact our pharmacy benefits manager CVS Pharmacy at Join CVS Caremark Network 
• Pharmacies and pharmacists providing DME and/or immunizations/POCT complete a contract request form. To enroll multiple pharmacists or pharmacists with multiple service locations, please use the excel please send an updated roster to ProviderRostersIA@MolinaHealthcare.com. Submit completed form(s) to IAProviderContracts@MolinaHealthcare.com.
• To add a new provider to an existing network practice group, please send an email with your updated roster to ProviderRostersIA@MolinaHealthcare.com. Email the completed form with supporting materials to IAProviderContracts@MolinaHealthcare.com.
