Note: To join Molina’s provider network, you must first be enrolled with and hold an active Medicaid ID from the Idaho Department of Health and Welfare.

• Complete a Molina contract request. Email your completed form with a current W-9 to MHIDProviderContracting@MolinaHealthcare.com.

• To add a new provider to an existing network practice group, use our Provider Roster Template and submit your changes to MHIDPNA@MolinaHealthcare.com.
