We appreciate your dedication to delivering quality care to patients, including those in our HealthChoice Illinois (Medicaid), MLTSS, Medicare-Medicaid (FIDE D-SNP), Medicare Advantage (MAPD) and Marketplace plans.

Note: To join our HealthChoice network, you must first be enrolled as an Illinois Medicaid provider and have an active Medicaid ID number from the Illinois Department of Healthcare and Family Services (HFS).

• Dental providers contact our vendor partner.  Note: If you are an oral surgeon or facility, please complete and submit Molina’s Contract Request Form to MHILProviderNetworkManagement@molinahealthcare.com
• Vision Providers contact our vendor partner
• Hearing Providers contact our vendor partner
• Pharmacy Providers contact our pharmacy benefits partner, CVS Pharmacy at Join CVS Caremark Network.
• All other providers complete a Molina Contract Request Form. Email your completed form with a current W-9 to MHILProviderNetworkManagement@MolinaHealthcare.com.
• To add a new provider to a participating group or PHO/PO, submit a Provider Information Update Form and updated roster to MHILProviderNetworkManagement@MolinaHealthcare.com.
