Medicaid (MSCAN, CHIP) and Medicare D-SNP

To join Molina’s MSCAN, CHIP and Medicare D-SNP network, you must first enroll as a Mississippi Medicaid provider, hold an active Medicaid ID number and be credentialed by the Mississippi Division of Medicaid (DOM).

• If you are a new provider, first complete enrollment and credentialing with DOM. Molina will be notified when your credentialing is complete, and our Provider Network Contracting Team will email you directly.
• If you are an existing provider with an active Medicaid ID already credentialed by DOM, complete a provider contract request form and return to MHMSProviderContracting@MolinaHealthcare.com
• If you are a provider joining a group with an existing Molina network contract, your group administrator can complete a Provider Information Update Form and submit it to MHMSProviderUpdates@molinahealthcare.com.

At Molina, we are always actively recruiting dental providers to enhance our statewide Mississippi Medicaid dental program network and ensure members have choice and access to quality dental care services. We have made the contracting proves quick and easy.

• You must be enrolled as a Medicaid provider and have an active Medicaid ID. Providers must credential/re-credential through the Mississippi Division of Medicaid medicaid.ms.gov/mesa-portal-for-providers/
• Review and sign the Dental Provider Service Agreement.
