Thank you for your interest in joining Molina Healthcare of Nebraska’s provider network. We appreciate your dedication to delivering quality care to patients, including those in our Medicaid and Medicare Dual Eligible Special Needs Plans (DNSP).
Note: To join Molina Healthcare of Nebraska’s network, you must be enrolled as a Nebraska Medicaid provider and have an active Medicaid ID number.
Vision providers can request a contract from our vision services vendor VSP or call 1-800-615-1883.
Hours of operation: Monday through Saturday, 8 a.m. to 6 p.m., CT, excluding major holidays
Pharmacy providers can contact our pharmacy benefits manager, CVS Pharmacy at Join CVS Caremark Network.
All other providers, including dental providers, can complete a Contract Request Form and submit it with an updated W-9 to NEContracting@MolinaHealthcare.com.
To add new providers to a participating group or PHO/PO, please submit a Provider Information Update Form to NEContracting@MolinaHealthcare.com.
