Thank you for your interest in joining the Molina Healthcare of Texas provider network. We appreciate your commitment to delivering quality care to members enrolled in our Medicaid (STAR and STAR+PLUS), CHIP, Medicare (DSNP), and Individual and Family (Marketplace) plans.

Note: To join Molina’s provider network, you must first be enrolled with and hold an active Medicaid ID from the Texas Medicaid and Healthcare Partnership Texas Medicaid and Healthcare Partnership.

Please provide the following information when submitting a request to join the Molina Texas network:

• A fully completed Contract Request Form (CRF)  
• A complete, current W-9
