Molina’s Provider Self Services has a variety of tools to simplify your transactions whether you need to check eligibility or check claim status. For more information contact Provider Services. Molina Healthcare is now collecting National Provider IDs (NPIs) from contracted and non-contracted providers. Submit your NPI to Molina via our Provider Self-Services Portal.

At Molina Healthcare, we consider ourselves a part of our member’s extended family. To our extended family, we are committed to providing high quality health care by having a robust network of physicians, hospitals, healthcare facilities, and other healthcare providers.

To join our network

Begin the process of joining our network of quality providers by completing a Provider Data Form and submit along with a W-9 to or by facsimile to (877) 814-0342.

We will evaluate your submission and provide you with a response within 90 days of the next steps.

Please Note: To be a contracted provider with Molina Healthcare of Washington, you must have your NPI registered with the Health Care Authority (HCA). If you have questions regarding this state requirement, please contact 1-800-562-3022 ext. 16137.

Washington law requires all health care providers submit credentialing applications through ProviderSource. To avoid delays please ensure applications are up to date.
