Before you contact Molina, you must have NPIs registered with the Health Care Authority (HCA).http://www.hca.wa.gov/billers-providers/apple-health-medicaid-providers/enroll-provider Please note, all required NPI numbers on a claim form must be registered with the HCA. If you have questions regarding this state requirement, please contact the HCA at 1-800-562-3022 ext. 16137.
Begin the process of joining our network of quality providers by completing a Contract Request Form and submit along with a W-9 to MHWProviderContracting@MolinaHealthcare.com.
Add a new provider to an existing group by completing an Add Provider/Mini Application form and return to MHWProviderContracting@molinahealthcare.com
Washington law requires all health care providers submit credentialing applications through ProviderSource. To avoid delays please ensure applications are current, including work history and attestations. You must also grant Molina access to retrieve this application.

We will evaluate your submission and provide you with a response within 120 days of the next steps. Incomplete submissions will not be evaluated.

Are your NPIs registered with HCA?
ALL required NPIs billed on a claim form for in-network providers must be registered with HCA in order to receive payment. These include the group billing NPI, the individual practitioner’s rendering or servicing NPI, and the attending practitioner’s NPI on a facility claim.
