SMMC Medicaid

The Florida Department of Children and Families (DCF) runs MyAccess, Florida’s SMMC Medicaid program.

You may qualify if you meet the following:

You live in Florida.
You are a U.S. citizen or a qualified immigrant.
You have a Social Security Number (SSN) or have asked for one.
You meet the income rules.

Some people qualify because of the following:

Age
Pregnancy
A disability
Need for long-term care

Where is Molina Healthcare of Florida SMMC Medicaid available?

To join a plan, you have to live in our service area. We have SMMC Medicaid plan in two Florida counties: Miami-Dade and Monroe.

CMS Plan

The population eligible to enroll in the CMS Plan will consist of Title XIX and Title XXI recipients with qualified conditions who are identified by the Florida Department of Health (FDOH).

Title 19: Children ages 0 -20 years of age that quality for Medicaid and ages 1 – 18 years with a qualifying condition are eligible for Title XXI (19).

Title 21: A child must meet clinical and Florida KidCare (CHIP) eligibility requirements to be enrolled in Title XXI (21). Florida Healthy Kids Corporation has the authority for determining eligibility for enrollment in Title XXI (21).
