Molina Healthcare wants you to get the care and support you need.If you are in our HealthChoice Illinois plan, you may be able to get waiver services. These services help you stay in your home and community.
Who can get waiver services?
The Illinois Department of Human Services (DHS) decides if you can get waiver services. DHS will meet with you and do an assessment called a Determination of Need (DON). This helps them see if you qualify. If you qualify, you may get services that help you live safely at home.
Waiver Programs
Waiver programs give you longterm help so you can live at home or in your community. Not every service is right for everyone. After you choose your services, Molina Healthcare will help set them up. Molina works with five waiver programs for eligible members.
