Grievances

If you have a problem or concern with your medical care or our services, you have the right to file a complaint. At Molina, we refer to a complaint as a grievance. Some examples of grievances are:

  • The care you get from your provider
  • The time it takes to get an appointment or be seen by a provider
  • The providers you can choose for care

Appeals

You can file an appeal when you do not agree with our decision to:

  • Change, suspend, or reduce a service
  • Stop or deny a service
  • Deny payment for services provided

How to file a grievance or appeal:

You can file a grievance or appeal by:

  • Phone. Call Member Services at (888) 483-0760 (TTY: 711) Monday – Friday 8 a.m. – 6 p.m. local time.
  • Mail. Fill out a grievance or appeals form and mail it to:

If you want a family member or your doctor to file the appeal for you, we need your written permission.
