Did you experience a problem with your quality of care? Did you get a decision that you don’t agree with? If you’re unhappy for any reason, we want to know.

Grievances
If you have a problem or concern with your medical care or our services, you have the right to file a complaint. At Molina, we refer to a complaint as a grievance. Some examples of grievances are:

  • The care you get from your provider
  • The time it takes to get an appointment or be seen by a provider
  • The providers you can choose for care

Appeals
You can file an appeal when you do not agree with our decision to:

  • Change, suspend or reduce a service
  • Stop or deny a service
  • Deny payment for services provided
