Make sure you live in our service area and have Medicare Part A and Part B. For D-SNP, you must also have Medicaid and be in a qualifying dual eligible category
How to enroll
Apply online, by mail or through Medicare.gov. For D-SNP, have your Medicare and Medicaid details ready.
Make sure to renew
D-SNP members must apply for Medicaid each year to maintain coverage. If you keep your Medicaid eligibility, your D-SNP renews automatically.
How Medicare eligibility works
To join a Molina Medicare plan in Arizona, you must live in our service area and have Medicare Part A and Part B.
For our D-SNP plan, you must also have Medicaid and be in a qualified dual eligible category. You, Medicaid or another party must continue paying your Medicare Part B premiums.
What you need to apply:
- Your Medicare number and Part A and Part B start dates
- Your Medicaid information if enrolling in D-SNP
- Your home address and contact information
