How Medicare eligibility works
Our Passport Advantage (HMO D-SNP) is a plan for people who qualify for Medicare and Medicaid. To join Passport Advantage (HMO D-SNP), you must live in our service area, have Medicare Part A and Part B and qualify for Medicaid. You, Medicaid, or another party must continue paying your Medicare Part B premiums.
What you may need to apply:
- Your Medicare number and Part A and Part B start dates
- Your Medicaid information if enrolling in D-SNP
- Your home address and contact details
