You may qualify if:

You live in a Molina service area, and
You have Medicare Part A and Part B
You have Medicaid or qualify for a Medicaid program that helps pay for Medicare cost

You can apply:

Have your Medicare info ready. If you are applying for D-SNP, also have your Medicaid details.

D-SNP members must keep their Medicaid each year. If you stay eligible for Medicaid, your D-SNP plan renews automatically.

How Medicare eligibility works

Molina Medicare Complete Care (HMO D-SNP) is a Medicare Advantage plan for people who have both Medicare and Medicaid. It includes all Original Medicare benefits and more.

You can join if:

You live in the plan’s service area
You have Medicare Part A
You have Medicare Part B, and you or Medicaid continue to pay your Part B premium, if required
You have Medicaid
Your Medicaid coverage qualifies you for a Dual Eligible Special Needs Plan (D-SNP)

1. Apply for and keep your Medicaid eligibility with the state each year.

2. If you stay eligible for Medicaid, you do not need to re-enroll in D-SNP. You will be automatically re-enrolled.

3. If your Medicaid provider is not Molina, you can still join our D-SNP plan.

Enrollment is subject to approval from the Centers for Medicare and Medicare Services (CMS). If your enrollment is not accepted by CMS, we will notify you.
