What Are Healthy Actions?
Healthy actions are routine screenings you complete with your doctor. These visits are important to keep you healthy or catch health problems early. They help your doctor come up with a care plan for you.

Annual Wellness Visit |
Yearly in-person check-up with your doctor or in-home visit. Talk to your doctor about your care plan and ask any questions you may have. |

Colon Cancer Screening: |
Screening to check for signs of colon cancer in adults ages 45-75. Eligible screenings include:

Breast Cancer Screening: |
Mammogram to check for signs of breast cancer in women ages 40-74. A mammogram is an x-ray of your breast. The test should only take a few minutes. Your doctor will use a special x-ray that will compress each breast. This may cause some discomfort but should not hurt. |

Diabetes Screenings |
Yearly diabetes screenings for adults ages 18-75 with diabetes. *You must complete all 3 screenings to claim your reward: The below screenings are listed as individual screenings on the website. This needs to be fixed. 1. A1c test: Blood test to check your blood sugar levels 2. Diabetic eye exam: Yearly exam with your eye doctor to check your eyes for signs of damage caused by diabetes 3. Diabetic kidney health test: A urine and blood test to check your kidney health |

Want to complete some screenings from the comfort of your home? Molina can mail you a home lab kit. The lab kits can check:

You must complete your health screenings before you claim your rewards. You can claim your rewards in one of the following ways:

How Can I Use My Rewards?
Your rewards will be loaded onto your Nations Benefits Mastercard Prepaid Card. You can use your rewards to buy healthy food and produce. For a list of products, click here.You can visit Molina.nationsbenefits.com to find stores that are part of the program.
What Happens to my Rewards if I Disenroll?
You must be an active Molina member to claim your rewards. You will lose your unspent rewards if you disenroll with Molina.
