Molina Healthcare routinely reviews approval rates for CPT and HCPCs codes submitted by Providers for those services requiring prior authorization. If a provider’s prior authorization approval rate meets a level set by the state, Molina may grant “Gold Card” status for those services and Providers will not be required to obtain prior authorizations for those services.
