Get support from a care manager

Living with health problems can be hard. We offer special care for members who need extra help with a health problem.

Care managers can help you:

-Make your plan for care

-Schedule a doctor’s visit

-Find support in your local area

If you live with life-long health issues, we can help you to get the care you need. You can also call Member Services at (844) 236-0894 (TTY: 711). You may opt out of this program at any time. Just call Member Services.

Our Community Connector Program is designed to help you:

• Find a place to live or get a ride to a doctor’s visit
• Find a medical home
• Find meals or food banks
• Renew your Medicaid coverage

HOME Project

Hope and Opportunity in Many Environments (HOME) is a project in Iowa where people helps others who need mental health care, support for a disability or help as an older adult. Learn more here

In Lieu of Services 

Along with the Iowa Department of Health and Human Services (Iowa HHS), Molina Healthcare of Iowa offers In Lieu of Services to Iowa Medicaid members who are on a waitlist for one of Iowa’s 1915(c) Home and Community-Based Services waivers.

This care supports those who are at risk of being admitted to a hospital or other kind of place of care. To help you stay at home, we offer care such as:

• Housing Support Services
• Home Changes
• Car Changes
• Case Management
• Respite Care
• Personal Care
• Medically Tailored Meals
• Assistive Care
• Intermittent Supported Community Living (SCL)
• Supported Employment
• Personal Emergency Response Systems
• Specialized Equipment
• Adult Day Care
• Rides to visits that are not your doctor Questions?
