Are you eligible for Molina Healthcare?

  • You may be eligible if you quality for both Medicaid and Medicare, but you are not part of the Medicare-Medicaid Alignment Initiative.
  • To enroll with Molina Healthcare under HealthChoice Illinois MLTSS you must live within our service area. Molina Healthcare will serve the state’s 102 counties beginning in 2018.
  • If you are eligible, you will have received information from Client Enrollment Services.
