Are you eligible for Molina Healthcare HealthChoice Illinois?

  • You may be eligible if: you are an older adult and/or an adult with a disability and eligible for Medicaid but not Medicare, you or your child is eligible for full Medicaid benefits.
  • To enroll with Molina Healthcare HealthChoice Illinois, you also need to live in our service area.
  • If you are eligible, you would have received information from Illinois Client Enrollment Services.

You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
