D-SNP Plan Replacing MMAI Plan

The Centers for Medicare & Medicaid Services (CMS) and the Illinois Department of Healthcare and Family Services (HFS) are ending the Medicare-Medicaid Alignment Initiative (MMAI) (also called MMP/Duals) on 12/31/2025. Molina Healthcare of Illinois is offering a dual-eligible special needs (D-SNP) plan in its place beginning 1/1/2026. This program benefits members by providing the convenience of coordinated care with one Primary Care Provider (PCP) and one ID card.
