Step 1: Go to www.mymolina.com (MiMolina.com en Espanol)
Step 2: Enter the last four digits of your Social Security number, date of birth and zip code
Step 3: Enter your email address
Step 4: Create a password
Step 5: You are now ready to log in and use MyMolina.com!

Want to access your care plan?
If you have a Molina Care Manager, you can also access your care plan in My Molina! Here’s how:
- Login to My Molina.
- Select “My Health Record.”
- Click on “Care Plan.”
