How to Enroll in a Molina Complete Care for MyCare Ohio (HMO D-SNP) Health Plan

What is MyCare Ohio?
MyCare Ohio is a program that helps improve the way your Medicare and Medicaid benefits work together.

How do I know if I am eligible for MyCare Ohio?
If you are eligible for MyCare Ohio, you will receive a letter from the Ohio Department of Medicaid telling you that you are eligible. Learn more about eligibility.

How do I choose a health plan?
Your eligibility letter will tell you when you must select a health plan to provide both your Medicaid and Medicare benefits. Molina Complete Care for MyCare Ohio is one of the plans you can choose. You have the right to choose the health plan that is best for you. However, if you do not choose a plan, one will be chosen for you.

What are the extra benefits of the Molina Complete Care for MyCare Ohio plan?

Molina Complete Care for MyCare Ohio offers extra benefits to our members:

  • Up to $6,000 each year for dental treatments like fillings, crowns, and dentures.
  • $0 hearing tests every year. And hearing aids covered every two years.
  • $300 each year to buy glasses, contact lenses, or special lens features like tinting..
  • Up to 104 one-way rides each year to places like your doctor, pharmacy, or food pantries.
  • Extras for caregivers, including care coaches and 40 extra hours of respite care.
  • $230 each month on a prepaid debit card for over-the-counter items.
  • Covered fitness options.

 

Also, the plan is designed to make your Medicaid and Medicare benefits work together better. That means one ID card, one toll-free Member Services phone number and a Care Coordinator to help manage your needed services.

Do I have to participate in MyCare Ohio?
If you do not want to get your Medicare benefits from a MyCare Ohio plan, tell a representative when you call the Ohio Medicaid Hotline. You will continue to get your Medicare benefits the same way you do today.

You will still be enrolled in a MyCare Ohio health plan for your Medicaid benefits. We call this plan Molina MyCare Ohio Medicaid.

Think carefully about choosing not to get your Medicare benefits from a MyCare Ohio plan. Molina Complete Care for MyCare Ohio has extra benefits for members who get both their Medicare and Medicaid benefits from us.
