Member Rights & Responsibilities

Below are the Passport by Molina Healthcare rights and responsibilities as written in the Passport Member Handbook. Kentucky Law requires that health care Providers or health care facilities recognize Member rights while they are receiving medical care and that Members respect the health care Provider's or health care facility's right to expect certain behavior on the part of patients.

What is a Dual Special Needs Plan (D-SNP)?

Thank you for caring for our D-SNP members. This type of Medicare Advantage plan provides health benefits for people who are “dually eligible,” meaning they qualify for both Medicare and Medicaid.

Anyone who meets the eligibility criteria for both Medicare and Medicaid is qualified to enroll in a D-SNP plan and many face unique health needs such as:

  • Have a disabling condition and struggle with daily activities such as bathing and dressing
  • Suffer from a cognitive impairment
  • Receive care from multiple doctors to manage a variety of health conditions
  • Require the assistance of in-home care providers or other health and social services
