Important Reminder:

It is important to Senior Whole Health and your patients that your provider directory demographics are accurate. Please visit our Provider Online Directory to validate your information and notify us if there are any updates. To make changes below please utilize the Provider Information Update Form.

Please notify Senior Whole Health of Massachusetts at least 30 days in advance when you have any of the following:

Change in office location, office hours, phone, fax, or email
Addition or closure of office location
Addition or termination of a provider
Change in Tax ID and/or NPI
Open or close your practice to new patients (PCPs only)

SWH collaborates with DirectAssure to help maintain a more accurate and timely directory. Providers may update information with CAQH ProView®, at CAQH ProView - Sign In. When updating your CAQH profile it is important to select "Global" for your access to ensure SWH can review these changes to your data. We encourage all providers to sign up for CAQH ProView® in order to utilize DirectAssure.
