2025 Molina Member Rewards Program!
Getting your Molina reward* just got easier! You can now submit your information through your member portal at MyMolina.com.
Molina's new reloadable reward card lets you continue earning rewards for completing important wellness visits and screenings. Shop in person at participating retailers.
Child Rewards
- $50 Reward – Well-Care Visit Ages 3-7 once per calendar year per child
- $50 Reward – Well-Care Visit Ages 11-21 once per calendar year per child
- $250 Instacart Code Towards a Bike and Helmet – Well-care Visit Ages 8-10 once per calendar year
- (bike incentive may be declined in exchange for a $50 gift card reward)
Cancer Screening Rewards
- $100 Reward – Breast Cancer Screening (Mammogram) Ages 50-74
Pregnancy Rewards
- 7 months of Instacart+ - $100 in Fresh Funds each month for 7 months (up to $700 total)

become pregnant within 42 days of joining Molina, may join this program. Members
must complete their first-trimester visit with their OB/GYN.

Claiming your rewards is easy!
- Complete a health screening(s) mentioned above.
- Log in to your member portal at MyMolina.com or download the app.
- LOG IN with your username and password or create an account.
- Navigate to My Wellness > My Healthy Rewards.
- Complete the form with all required information.
- Have the date of your exam ready.
- Choose your reward
- Submit the reward form
Reward Delivery:
- For child and cancer screening rewards, you will receive your reloadable reward card in the mail with additional information in about two to four weeks.
- For Instacart rewards, you will get your reward sent to the email listed on your Molina account within one to two weeks.
*Important Information:
- To earn a reward(s), you must have Molina Healthcare of Arizona as your primary insurance when service is given.
- You must see your doctor to earn these member rewards.
- All children need to visit a doctor or pediatrician to qualify.
- Women need to get a cervical cancer screening from their OB/GYN or doctor to qualify.
- Pregnant women need to see an OB/GYN or primary care doctor during their first trimester to qualify.
- All reward submissions for services done in 2025 must be submitted by 1/31/2026. Submissions received after the deadline will not be processed.
