Personal Attendant Services
Personal attendants are caregivers that help with activities of daily living and house tasks needed to keep a clean and safe home environment in community settings (except in adult foster care homes or assisted living facilities). Services may have protective supervision and help do health-related tasks given by a registered nurse. The attendant helps with house work items such as shopping, light housework and laundry. The attendant can also help with personal care items such as bathing, dressing and feeding.

Day Activity and Health Services
Licensed day activity health services (DAHS) facilities provide daytime care to members who live in the community. Services are provided Monday to Friday and members can get care up to 5 days per week. They provide for physical, mental, medical and social needs. Services include nursing and personal care, noon meal and snacks, transportation, and social, educational and recreational activities. Sometimes this is called adult day care or adult day services.

Nursing
This service focuses on long-term needs rather than short-term acute healing needs, like weekly insulin syringes or pill box set up for members cannot do this for themselves. The goal is to get you to your highest level of  highest level of health and functioning. These services are given instead of hospitalization or a nursing facility stay. A doctor’s order is needed for this service.

Emergency Response Services
This service includes electronic equipment that allows members 24-hour access to help in an emergency. The equipment is connected to your phone line to call the response center and/or other forms of help once the button is pressed.

Home Delivered Meals
This service includes ready to eat food brought to the member’s home that may consist of a heated lunch, dinner or both. This service is for the member who cannot make his or her own meals but is able to feed him or herself.

Minor Home Modifications
These are physical modifications to a member’s home. The modifications must be needed to support the health, well-being, and safety of the member while helping them function with more independence in their home. Without the modification, a member would need to live in an institution like a nursing facility or assisted living facility. Changes to a home that do not help the member’s safety or independence are not included as part of this service, like new carpet, roof repair, central air or home additions.

Adaptive Aids, Medical Equipment and Medical Supplies
This service includes devices, controls, appliances or medical equipment and supplies that help people live more independently. These will help the member to raise his or her abilities to perform activities of daily living, or to communicate with others.

Dental
These services are provided by a dentist to care for teeth and meet related medical needs. Services do not include cosmetic dentistry.

Physical, Occupational and Speech Therapy
These services help improve and/or restore a person's functioning: includes physical, occupational and/or speech therapy. A doctor’s order is needed for this service.

Adult Foster Care
A 24-hour living arrangement in an adult foster care home enrolled with DADS. Services may include meal preparation, housework, personal care, and help with transportation and activities of daily living. Members pay room and board.

Assisted Living Facility
This service is a 24-hour living arrangement in a licensed personal care home. Services may include personal care, home management, social and recreational activities, 24-hour supervision, help with the administration of medicine, and transportation. Individuals pay room and board.

Transition Assistance Services
These services help people go from a nursing home or institutional setting into a home in the community by paying for one-time costs of setting up their new home.

Financial Management Services
These are services that help people using the Self Directed Services option to manage their payroll and budget.

Respite
This service provides help for unpaid family or primary caregivers who are meeting all the needs of the member. The respite caregiver helps the member with all daily needs when the family or primary caregiver is absent. Respite can be give in-home or out-of-home.

Protective Supervision
Protective supervision is watching over the member while the primary caregiver is out of the home. It does not require the delivery of personal care tasks. Protective supervision is needed to protect the member from injury due to his/her memory loss and/or physical weakness.

Cognitive Rehabilitation Therapy
This therapy can help a person learn or relearn cognitive skills that have been lost. This may be the result of damage to brain cells. It can be offered in an outpatient setting or in your home. You can speak with your doctor to find out if you can be tested and if you are able to get this service.

Employment Assistance
This service helps you find paid work in the community by helping you figure out your interests, and skills. This service also helps you learn about what support you may need for your work. 

Supported Employment
If you have a job, this service gives you support to help you keep your job. Services include helping you adapt to your work setting and with supervision and training, based on your support needs.
