Home and Community-Based Services (HCBS)

Children’s Home and Community-Based Services are strength-based services for children and youth up to age 21 who have serious emotional difficulties, developmental difficulties and/or are medically fragile. The services are intended to support children and their families or caregivers where they are most comfortable – at home or in the community. Eligibility for these services is determined by an assessment conducted either by a Children’s Health Home or New York State’s Independent entity, C-YES. These services include:

  • Caregiver/Family Support & Services
  • Respite (Planned or Crisis)
  • Vocational Supports
    • Pre-Vocational Services
    • Supported Employment
  • Community Self-Advocacy and Training
  • Adaptive & Assistive Equipment
  • Vehicle Modifications
  • Environmental Modifications
  • Palliative Care
    • Expressive Therapy
    • Massage Therapy
    • Pain & Symptom Management
    • Bereavement Services
  • Non-Medical Transportation
