As a ForwardHealth member, one of the most important things you can do is to keep your contact information up-to-date. It will help ensure you’re able to get the care you need when you need it. It also helps make sure you get important updates and reminders from us. 

You also need to update your contact information with ForwardHealth. This helps keep your benefits active.

Your contact information includes your:

  • Phone number
  • Email address
  • Mailing address

 

Step 1: Update your contact information with Molina by:

  • My Molina mobile app
    • Download My Molina mobile app from the Apple App Store or Google Play Store. You can also scan the QR code below using your smart phone.
      My Molina Mobile App
    • Register or log in with your Molina member ID number.
    • Go to Menu > Communication Preferences.
    • Add/edit your phone number, email and mailing address.
    • Choose your preferred phone number and email to receive communications from Molina.
  • My Molina member portal
    • Go to MyMolina.com
    • Register or log in with your Molina member ID number
    • Click on the Communication Preferences button.
    • Add/edit phone number, email and mailing address.
    • Choose your preferred phone number and email to receive communications from Molina.
  • Member Services
    • Call us at (888) 999-2404, TTY: 711 and we’ll update your contact information for you.

*Texts from Molina would include notification the Molina newsletter is available online, changes made to the member handbook, flu shot reminders, notification to renew your benefits, alerts about health-related community events, member surveys and health emergency updates.

You can stop receiving Molina texts at any time. To do that, click the unsubscribe link at the bottom of the message you receive or make a phone call to Molina Member Services to unsubscribe. Once you unsubscribe to text, remember Molina can only mail you Molina information based on the mailing address you have provided the state.
