Complete your 2026, health screening(s) before December 31, 2026, to receive your gift card. Complete the form(s) below after receiving your services. Follow the instructions on the form(s) to send them to Molina. Molina will verify all requests for gift cards through claims data. It may take up to 6-8 weeks, or longer, to receive your gift cards.
This reward program is for HealthChoice Illinois members. Members must be enrolled with Molina at the time of service to qualify. Services must be completed in 2026.
Members do not automatically receive a gift card after their service(s). Members, or providers, must claim rewards by completing one of the forms below. Members may also call Member Services to claim reward(s).
