Apple Health (Medicaid) Member Rewards

Get your flu vaccine (shot) and earn a $50 Reward! You must receive your flu vaccine between September 1, 2025, and April 30, 2026, to be eligible for this reward. This offer is available to Apple Health (Medicaid) members ages 6 months and older.

After you or your child get your flu shot, let us know by providing your details using the member rewards submission process mentioned below. It's that easy!

Note: Molina Apple Health members ages 6 months and older are eligible for this reward. To earn the reward, you must have Molina Healthcare of Washington as your primary insurance at the time of receiving your flu vaccine. You have until May 31, 2026, to submit your vaccine details.

Claiming your reward is easy!

  1. Complete one or more of the health screening(s) mentioned above.
  2. Log in to your member portal at MyMolina.com.

    Note: Rewards can be submitted either under an individual's member portal account or under the primary insurer's account. If you do not have an account, you will need to create one or call Molina Member Services at (800) 869-7165, TTY: 711 for help claiming your reward.

  3. Click on the "My Wellness & Healthy Rewards" tab, then click the "My Healthy Rewards" tab.
  4. Click on the "Request Submission" button to view the secure online form.
  5. Fill out the form with details of the health screening(s) you have received. You can submit multiple screenings at the same time.
  6. Click the "Submit" button, and you're done!

 

Reward Delivery:

You will receive your reloadable reward card in the mail in about 2 to 4 weeks. If you have already received your card, it will be reloaded with the reward amount in 1 to 2 weeks.

Important Information:

  • To earn a reward(s), you must have Molina Healthcare of Washington as your primary insurance when service is given.
  • Apple Health members can get up to $200 in total rewards every 12 months.
  • Each reward can be received no more than once a year. For example, a diabetes HbA1c test can be completed every 3-6 months, but you can receive only one $35 reward each year.
  • All reward submissions for services done in 2026 must be submitted by 1/31/2027. Submissions received after the deadline will not be processed.
