Your Member Identification (ID) Card

When you become a member, you get a Member Identification (ID) Card in the mail from Molina Healthcare.

Be sure to carry your Member ID Card with you at all times

  • It is used by health care providers, such as your behavioral health (mental health and drug and alcohol treatment) provider and hospital to determine if you are eligible for services through Molina Healthcare.
  • You must show it every time you get behavioral health care.
  • If you lose your Member ID Card, you can get a new one this way:
You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
