Molina Healthcare of New York Child Health Plus (CHPlus) Eligibility

Child Health Plus (CHPlus) is for children between the ages of 0 and 19, residing in New York state and who do not qualify for Medicaid. CHPlus eligibility in New York is determined by the New York State Department of Health.
