Senior Whole Health NHC (HMO D-SNP) is a comprehensive Medicaid Advantage Plan (MAP) for adults 18 or older who are eligible for long-term care and have both Medicaid and Medicare
- You reside in the Senior Whole Health of NY NHC (HMO D-SNP) geographic service area
- – and – you must have both Medicare Part A and Medicare Part B
- – and – you must be eligible for Medicare and Full Medicaid
- – and – you must be enrolled in our Medicare Advantage program.
